Your Privacy  Español

Protecting your privacy is very important to CHNCT. We have many procedures in place and have talked with our employees to be sure we protect (keep safe from harm) the health information we have about you. Some examples of health information we have about you are the following:

• Your name, address and telephone number;
• Your HUSKY Identification number;
• Your social security number; and
• Information we receive from your doctors when they bill us for the services they provided to you or when the doctor or doctor’s office sends a referral to us.

The Connecticut Department of Social Services (DSS) has created a Notice of Privacy Practices that was sent to all CHNCT members. DSS needs to send this Privacy Notice to you because the law requires it. This Privacy Notice tells you how health information we have about you is shared for our normal daily operations (for example, when your doctor bills us for services provided to you and we pay for those services, we are sharing your information) and when we are required by law to share it. It also tells you what your privacy rights are. As a member of CHNCT, you have the privacy rights listed below:

• You have the right to ask us to make a change to health information that we created about you if it is wrong.
• You have the right to ask that we send certain information to you in a confidential way if your life would be in danger if we sent them to your normal address.
• You have the right to ask for a copy of certain health information we have about you.
• You have the right to get a listing of certain times when we have shared your health information with others.
• You have the right to complain to DSS or the Department of Health and Human Services (DHHS) if you feel your privacy has been violated.
